This is an excellent opportunity for a bilingual MA who thrives in a patient-centered environment and is comfortable working independently from home.You will serve as a key touchpoint for our chronic disease patients — conducting monthly care coordination calls, documenting clinical interactions, updating care plans, and ensuring patients remain engaged with their treatment goals between office visits. This is a remote position requiring reliable internet, a quiet workspace, and strong self‐discipline.Key ResponsibilitiesConduct monthly CCM/PCM outreach calls with enrolled patients to review symptoms, medications, and care plan progressAccurately document all encounters and time logs ineClinicalWorks (ECW)Coordinate care between specialists, PCP, and ancillary providersAssist with care plan creation and updates under provider supervisionIdentify and elevate urgent patient concerns to the clinical teamMaintain compliance with CMS billing requirements for CCM/PCM time trackingSupport patient enrollment and consent documentationRequirementsMedical Assistant certification (CMA, RMA, or equivalent) preferredMinimum 2–3 years of experience in a clinical or care coordination roleProficiency ineClinicalWorks (ECW)— required, no exceptionsExperience withCCM, PCM, or RPM programsstrongly preferredBilingual — English/Spanish required(spoken and written)Must reside in theSpring, TXorKaty, TXareaReliable home office setup with stable internet connectionStrong communication, documentation, and time‐management skillsAbility to work independently with minimal supervisionCompensation & SchedulePay: $17–$25/hour depending on experienceFull-time or part-time options availableMonday–Friday | Standard business hours (CST)BenefitsPaid time offFlexible schedulingGrowth opportunities within an expanding practiceSupportive clinical team and clear workflowsFlexible work from home options available.#J-18808-Ljbffr
