Case Manager Social Worker (SDOH)

Position Summary:Position SummaryThe Social Worker will play a vital role in addressing social determinants of health (SDOH) that impact patients' well-being and health outcomes. This position is responsible for responding promptly to patients who screen positive for SDOH factors during admission and providing appropriate resources, referrals, and interventions to improve overall health and quality of life. Respond in a timely manner to SDOH alerts or positive screening results identified during patient admission. Provide education, counseling, and linkage to community resources and support services. Document interventions, referrals, and patient outcomes accurately and in compliance with regulatory and hospital standards.Participate in quality improvement initiatives and data tracking related to SDOH interventions and outcomes.Position Qualification:Education: Social Work degree or health science degreeExperience: Prior hospital, healthcare, or community social work experience preferred.Skills:* Strong assessment, problem-solving, and communication skills* Knowledge of community resources and support programs* Ability to work collaboratively within a multidisciplinary team* Empathy and cultural sensitivity
