Your MissionAs an Archer Health PT, you are the architect of a patient's recovery. You will provide skilled therapy services in the most meaningful environment possible-the patient's home. Key Responsibilities:Comprehensive Assessment: Evaluate therapeutic, rehabilitative, and functional status; assess muscle strength, mobility, gait, and ROM.Plan of Care: Develop and implement individualized treatment plans in collaboration with physicians and our interdisciplinary team.Home Environment Safety: Evaluate the home for hazards and recommend modifications or assistive devices to ensure patient safety.Education: Instruct patients and caregivers in therapeutic exercises and the use of appliances (orthotics, prosthetics, etc.).Supervision: Oversee Physical Therapy Assistants (PTAs) and Home Health Aides (HHAs) to ensure a cohesive care journey.Clinical Excellence: Maintain accurate, timely documentation (point-of-care) that reflects the high standards of our 5-star rating.The "Archer" MindsetWe are looking for therapists who are ambitious, empathetic, and driven by clinical results.Qualifications:Education: Graduate of an accredited Physical Therapy program (DPT preferred).Licensure: Current, unrestricted California Physical Therapist License.Certification: Current BLS/CPR Certification.Experience: Previous home health experience is highly preferred; experience with high-acuity patients is a plus.Mobility: Valid CA Driver's License with a clean driving record and reliable transportation.Communication: Exceptional interpersonal skills and the ability to collaborate within a multidisciplinary team.Ready to lead the future of home health?Join the agency that is setting the standard
