We work with patients of all ages and backgrounds who require daily care in their homes. Most of our patients have physical limitations so we hire the most compassionate and thoughtful talent in the field. You will be responsible for managing care for multiple clients, so time management skills are essential. It is important to provide each patient with the time and attention necessary to meet his or her needs. If you enjoy working one-on-one with patients outside of a clinical setting, then this could be the right career for you!Home Health Nurse responsibilities:Assess patients' condition during every visit and chart your observationsPerform evaluation tasks, including vital signs and medication reviewAdminister medication as prescribed by the patient's PhysicianDress or redress open wounds and assess progress of healingEducate patients and their families on proper home health care strategies and proceduresCoordinate with Home Health Aids, Physicians and anyone else involved in the patient's care planProvide palliative care as needed to keep the patient comfortableMake recommendations for devices or tools that might improve the patient's quality of lifeListen to the patient and respond to concerns or requestsAccurately and timely completion of medical record documentationEducation and Experience Equivalencies:RN/LPN License in SD requiredApplicable experience strongly preferred in home health setting.Applicants with excellent abilities in communication via email, Word, andmedical records preferred.Excellent time managementAll candidates must have (or qualify to obtain) a current SouthDakota driver's license, proof of insurance and be cleared through the Division ofCriminal Investigation.Physical Requirements:? Ability to perform duties as outlined above including lifting 10-20 lbs.? Continual standing and walking during shift.? Continual talking in person or on the phone during shift.?
