RN Case Manager-{169878} Job Title: RN: Case Manager Location: Jeffersonville, IN 47130 · Work Area/Coverage: Harrison, Clark, Crawford, Orange, partial Floyd & Washington counties (plus additional areas as needed for PTO/call-ins/admissions) Pay: $56.14/hour · Mileage Reimbursement: $0.46/mile Schedule: Monday-Friday, 8:00am-5:00pm + weekend on-call rotation Overview We’re hiring an RN Case Manager to complete comprehensive assessments for high-risk members and coordinate care across providers and settings. This role focuses on care planning, care transitions, patient advocacy, and driving quality outcomes through effective clinical case management. Key Responsibilities Perform comprehensive assessments of high-risk members Collaborate with primary care providers to implement an individualized care plan Identify, address, and continuously reassess appropriate levels of care Coordinate care transitions with physicians, social workers, discharge planners, and other stakeholders Put non-medical supports in place to support treatment plan compliance (e.g., transportation/housing resources as applicable) Maintain detailed documentation of clinical, functional, and fiscal outcomes Required Skills & Qualifications Active RN license in Indiana (compact license accepted) Home Health experience (2+ years recent) HCHB (Homecare Homebase) & OASIS experience Strong clinical skills including: Wound care, IV therapy, Drain care, G-tubes, Patient education Keywords RN Case Manager, Home Health RN, Care Management, Care Coordination, OASIS, Wound Care, IV Therapy, Drain Care, G-Tube, Patient Education, HCHB, Homecare Homebase, On-Call Rotation, Mileage Reimbursement
