Registered Nurse - Intake / Triage Hospice
This position is responsible for providing clinical triage support to patients, families, and caregivers as well as supporting the hospice field staff. This position also reviews clinical documents including medication records and referral information as part of the hospice intake process as well as providing phone support in the hospice office as needed.This is a hybrid position with 2 days/month in office. Candidates must reside in Portland Metro.
