In your role, you'll evaluate patients in both inpatient and outpatient settings, creating tailored treatment plans that cater to individual needs. You will implement targeted physical therapy interventions focused on mobility, strength, balance, and overall functional abilities. By collaborating closely with a multidisciplinary team including nurses, physicians, and other rehabilitation professionals, you'll work to optimize patient outcomes. Educating patients and their families about therapeutic goals, discharge planning, and home exercises will be an essential part of your role, ensuring they continue their journey to recovery outside of the facility. The job also believes in meticulous documentation of evaluations, treatments, and patient progress, kept in line with facility guidelines and regulatory standards. Participate in care team rounds and interdisciplinary conferences to ensure that everyone is aligned and working towards the best possible outcomes for our patients. To qualify, you will need a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program, alongside state licensure as a Physical Therapist. A CPR certification is preferred, and while specialized certifications can enhance your profile, they are not a strict requirement. Previous experience is strongly preferred, as well as an ability to treat patients facing complex medical and mobility challenges.
