Candidates may apply if have some college plus 4 years experience working in direct care services. *********************************************** some job duties include: *Complete assessments on referrals - identifying needs *Provide case management in the home, office or other natural setting. This may include linking to community resources, assisting with employment and housing needs; supervising visits; foster parent support; teaching parenting skills, etc *school based services (individual and or groups) working with kids who have been identified as needing extra assistance. *Document all contacts and complete progress notes for services *Prepare reports and communicates with collateral contacts and placing agencies *Develop a treatment plan *Attending family team meetings *Participate in supervision/staffing with supervisor ******************************************* Openings in Vigo, Sullivan, Parke, Vermillion and Clay counties ******************************************* We offer generous pay and other benefits.
