Join our dynamic Day Program as a Qualified Developmental Disabilities Professional (QDDP) — a role dedicated to empowering individuals with developmental disabilities through compassionate, person-centered support. This position offers the exciting opportunity to work during daytime hours with no on-call responsibilities and absolutely no weekends! You’ll be a vital part of a team committed to enhancing quality of life, promoting independence, and fostering community engagement. Duties Develop, implement, and monitor individualized care plans tailored to each individuals' needs. Assist individuals with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or other assistive devices, and assisting with meal prep including G-Tube feedings. Administer medications accurately following medical orders while maintaining compliance with HIPAA regulations and documentation standards Conduct regular assessments of individuals’ health status by monitoring vital signs and observing behavioral changes while documenting and charting the assessments on a regular basis. Collaborate with interdisciplinary teams including nurses, social workers, case managers, and families to coordinate comprehensive care strategies Manage behavioral challenges through behavior management techniques and conflict resolution skills to create and keep a safe environment Maintain detailed documentation within Electronic Medical Record (EMR) systems, including care plans, incident reports, and progress notes Support individuals during community outings or transportation by driving safely and adhering to all safety protocols Maintain accurate, current, and timely daily and monthly documentation. Requirements Proven experience working with individuals with disabilities Knowledge of medical terminology, medication administration procedures, and hospice or memory care practices Familiarity with EMR systems for accurate documentation and case management Ability to perform heavy lifting safely when assisting individuals with mobility or transfers using Hoyer lifts or similar equipment Valid driver’s license with a clean driving record for transportation responsibilities Certification in first aid and CPR required Strong interpersonal skills to effectively communicate with residents, families, and team members while maintaining confidentiality per HIPAA guidelines Flexibility to adapt quickly to changing resident needs and work collaboratively within a team environment Previous QDDP experience in a Day Program preferred Bachelor’s Degree or Equivalent in experience Join us in creating a nurturing community where every individual receives respectful, person-centered care.
