Job Opportunity: Occupational Therapist for In-Home Patient CareWe are looking for a dedicated and empathetic Occupational Therapist to join our Healthcare & Medical Services team, focusing on in-home patient care. In this role, you will provide personalized, evidence-based therapeutic interventions directly in patients' residences, aiming to restore functionality, improve safety, and encourage independence in daily tasks.Key ResponsibilitiesConduct thorough evaluations to assess patient needs.Develop and implement care plans that are goal-oriented.Deliver hands-on therapy that emphasizes functional mobility, self-care, home safety, and adaptive techniques.Work collaboratively with a multidisciplinary team, including nurses, physical therapists, speech therapists, social workers, and physicians.Coordinate patient care, track progress, and adjust treatment plans as necessary to achieve the best outcomes.Educate patients and their caregivers on home exercise routines, energy conservation, safe transfer methods, the use of adaptive equipment, and strategies to prevent falls.Maintain accurate documentation of all care provided in the electronic medical record, ensuring compliance with regulatory standards and agency policies.Qualifications and RequirementsValid Occupational Therapy license.Experience in home health care is preferred.Strong communication and interpersonal skills.Ability to work independently while being part of a supportive team.Working ConditionsThis position allows for autonomy in the field while being backed by a collaborative, patient-focused team.
