Program Coordinator

Duties and Responsibilities: The In-Home Program Coordinator or designee has responsibility for overseeing the day-to-day operations of residential in-homeproviding support services to the individual and DSP’s.  This position is responsible for providing supervisory oversight of program, facilities, and staff in accordance with agency policies and procedures, licensing standards, and accreditation standards.  In-Home Program Coordinator responsibilities include the following:Monitor and ensure documentation, records management, and coordination of services with CSB, etc. comply with licensing and accreditation standards.Ensure quarterly reviews of support plan are completed in a timely manner.  Ensure individuals’ records are maintained in accordance with licensing and accreditation standards.Ensure all individual’s documentation are in compliance with in-home regulations and standards.Provide regular and consistent supervision and support to In-Home staff.Conduct audits of in-home individuals to ensure all documentation are present for and up-to-date. Ensure compliance with agency and licensing regulations as it relates to training and certification.Coordinate training opportunities for the staff.Complete annual training reviews and re-certifications, including collecting all required documentation for DSP’s.Maintain up to date and accurate files on all DSP’s, including initial application, interviews, trainings, and annual re-certifications. Coordinate orientation & training for DSP’s.Conduct & facilitate intake, admissions for individual’s entering the In-Home Program. Provide crisis intervention and supportive encouraging.Attend team meetings as scheduled.Be personally familiar with each individual and family receiving services.Ensure that all required licensure, programmatic, and national accreditation regulations, rules, and standards are met in the Residential In-Home Program. Assist in visits by authorities as required. Encourage opportunities for individuals, family visits and interactions with SC’s, governing bodies and other agencies as needed.Serve as role model for individuals and staff.Perform other duties or tasks as assigned.MINIMUM QUALIFICATIONS:1 year of experience working in the field of developmental disabilities.Experience in the operation of sponsored placement residential services is preferred.Provide “TB FREE” documentation.Valid Driver's License and acceptable driving record.
