Our team is the heart of Cabrillo.Position: Licensed Vocational Nurse (LVN) Reports To: Team LeaderAbout The RoleWe’re looking for an LVN who is compassionate, reliable, and passionate about patient care. In this role, you’ll provide direct care under the guidance of a Registered Nurse, follow the plan of care, and work closely with our interdisciplinary team to ensure each patient receives the support they need.What You’ll DoProvide hands-on care to patients as allowed by the State Nurse Practice ActCarry out care plans based on assessments and patient needsDocument visits clearly and on timeHelp educate patients and families so they feel informed and supportedCommunicate patient needs to the wider hospice team (aides, therapists, social workers, dietitians, supervisors, etc.)Use equipment and resources safely and responsiblySupport pain and symptom management, and monitor how patients respondParticipate in training, development, and quality initiativesAssist with other tasks as requested by the RNWhat We’re Looking ForGraduate of an accredited LVN/LPN programAt least three years of nursing experience (hospice, community health, or med/surg is a plus)Active LVN/LPN license in the stateStrong communication and organizational skillsValid driver’s license and a reliable, insured vehicleCurrent CPR certificationA genuine understanding of the hospice philosophy and the needs of the terminally illHourly range for posted region: $35 USD - $45 USD
