CHM Mobile Care, based in Shelbyville, KY, is seeking a qualified board-certified physician for a contracted 1099 opportunity in the Louisville, KY area. In this role, you will provide primary patient care both in person and via telehealth for patients in a post-acute care facility and for patients transitioning from acute to post-acute care. You will work in collaboration with Advanced Practice Nurses to ensure each patient is provided the best care possible meeting safety and healthcare standards.To qualify for this role, you will have:MD/DO from an accredited university.Board Certification in Family or Internal Medicine.Active and unencumbered license to practice medicine in Kentucky.Active and unencumbered DEA license to prescribe in Kentucky.Active BLS Certification.Ability to see patients at a minimum of 2 days per week.Excellent communication and collaborative skills.Experience providing primary care for patients.
