Psychiatric Case Manager

Job Title: Traveling Psychiatric Case Manager Location: Houston, Texas Company: NP Care Services Group Job Description: We are seeking a dynamic and compassionate Traveling Psychiatric Case Manager to join our team in Houston, Texas. As a key member of our mental health services provider, you will be responsible for providing comprehensive case management services to individuals with psychiatric needs across various locations in Houston and surrounding areas. Responsibilities: 1. Conduct thorough assessments of clients' mental health needs and develop individualized care plans. 2. Coordinate and monitor psychiatric treatment plans in collaboration with healthcare providers, ensuring continuity and quality of care. 3. Provide crisis intervention and support to clients in need of immediate assistance. 4. Advocate for clients' rights and access to appropriate mental health services. 5. Facilitate referrals to community resources and support services to enhance clients' well-being. 6. Maintain accurate and up-to-date documentation of client interactions and progress. 7. Collaborate with interdisciplinary teams to ensure holistic care for clients. 8. Travel to various locations within Houston and neighboring areas to meet with clients and attend meetings. Qualifications: 1. Bachelor's or Master's degree in Social Work, Psychology, Counseling, or related field. 2. Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) preferred. 3. Experience in psychiatric case management and working with individuals with mental health disorders. 4. Strong communication, organizational, and interpersonal skills. 5. Ability to work independently and as part of a team. 6. Valid driver's license and willingness to travel within the Houston area. 7. Knowledge of community resources and mental health services in the Houston area.
