Knowledge of and delivers appropriate age-related care related to the physical and psychological needs of adult and geriatric residents per plans of care. An essential communication link with resident assistants. Provides hands on care to residents and works with additional resources to plan and deliver care for all residents in their household. Plays a key role in the resident admission process, orientation of new staff and students, and data collection on performance indicators. Performs tasks delegated by the supervising nurse as well as other managers in the blended household. Provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to assigned resident. Responsible for maintenance of the household environment during his/her scheduled shift. Assists the resident in transferring, re-positioning, and walking using correct and appropriate transfer techniques and equipment and provides range of motion and passive exercises. Assists residents with meals and snacks, provides water and supplements as appropriate. Administers prescribed medications
