If you're looking to advance your career in a supportive environment that values excellence, autonomy, and teamwork, we'd love to meet you.Position: Physical Therapist (PT)Pay Range: $55.00-$70.00/hrSign On Bonus: $5,000Status: Full-TimeCoverage Area: Coachella ValleyWhat You'll DoThe Physical Therapist (PT) is responsible for delivering high-quality physical therapy services in accordance with the physician's plan of care and all requirements outlined in the Personnel Services Agreement. The PT plays a key role in patient recovery, functional improvement, and interdisciplinary care coordination.Provide skilled physical therapy services based on the written physician's plan of careConduct comprehensive, ongoing assessments and collaborate with the patient, caregiver, and interdisciplinary team to update and individualize the plan of careEducate and counsel patients, caregivers, and families on therapy goals, safety, and home exercisesPrepare accurate and timely clinical documentation, including visit notes and progress summariesCommunicate regularly with the physician and healthcare team regarding patient progress and changes in conditionInitiate and implement the physical therapy program, instructing caregivers and appropriate staff on techniques they can safely performParticipate in case conferences to review goals, progress, and care plansTrain patients in the proper use of prosthetics and assistive devicesIdentify needs for additional home health services and coordinate referrals when appropriateParticipate in patient discharge planning and recommendations for continued careProvide in-service education and training to agency staff as neededEngage in peer consultation and quality improvement processesSupervise Physical Therapist Assistants (PTAs) in accordance with state and organizational guidelinesRequirementsActive CA Physical Therapy LicenseStrong verbal and written communication skillsStrong organization skillsPossesses and maintains current CPR certificationMust be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working orderWhy Join Bella Terra Home Health & Hospice of the Desert?Bella Terra is part of the Cornerstone Group, a nationwide network of 75+ home health and hospice agencies.
