Transition Specialist
Title: Transition Specialist Location: Indianapolis, IN (Hybrid) Duration: 8+ months Role Overview The Transition Specialist provides oversight and coordination of transition and reentry practices across Indiana's juvenile correctional facilities. This role focuses on preparing youth for successful reintegration into the community through structured programs, partnerships, and individualized support. Key Responsibilities 1. Transition Program ImplementationFacilitate the delivery of the Next Steps life skills curriculum.Utilize the Transition Coordinator Handbook to standardize transition practices across facilities.Evaluate and monitor the completion of five required Transition Consultations for each student.2. Reentry Planning & Case CoordinationParticipate weekly in Administrative Release Committees (ARC) to review and reinforce student reentry plans.Assist in developing individualized Education Learning Plans (ELPs) upon student intake.Ensure each youth has a structured and actionable transition plan.3. Post-Release SupportConduct follow-up calls with released youth at:1 month3 months6 months9 months12 monthsMonitor progress and provide guidance to support successful reintegration.4. Interagency Collaboration & PartnershipsCollaborate with state, county, and private agencies to provide comprehensive reentry services.Support partnerships with:Indiana Department of Workforce DevelopmentHIRE ProgramIndiana University HOPE Mentoring ProgramRepresent the Division of Youth Services at conferences, meetings, and community events.5. Education & Career ReintegrationAssist youth in transitioning back to public schools or alternative education programs.Provide guidance on:Employment opportunitiesApprenticeship programsMilitary service pathways6. Documentation & Life Readiness SupportHelp youth obtain essential documents, including:Social Security cardsState IDDriver's licenseBirth certificatesSupport overall life readiness and independence skills.Required Skills & QualificationsExperience working with youth, preferably in juvenile justice, education, or social services.Knowledge of reentry and transition planning practices.Strong communication and interpersonal skills.Ability to collaborate with multiple stakeholders and agencies.Organizational and case management skills.Preferred QualificationsExperience in correctional or rehabilitation environments.Familiarity with workforce development programs.Background in counseling, social work, or education.Sot SkillsEmpathy and ability to work with at-risk youth.Strong problem-solving and decision-making skills.Ability to manage sensitive situations with professionalism.Effective communication across diverse groups.Required skills:Experience utilizing strong communication skills (written and verbal).Proficiency in Microsoft Word, Outlook, Excel, and PowerPoint.Experience working in a customer service role.Experience in data entry.Experience working in a team environment.Experience working with at-risk youth.Experience in community or state programs.Experience working in youth educational programs
