What You'll Do:
Provide hands-on nursing care in accordance with scope of practice
Collaborate with RNs and physicians during rounds and care planning
Coordinate care and communicate changes to residents' families and team members
Administer medications and treatments, and monitor for effectiveness
Review and implement physician orders; assist in emergencies, including CPR
Document all care accurately and timely in the medical record
Complete admission documentation and assist in discharge planning
Ensure safe, clean, and well-stocked medication and treatment carts
Participate in medication management and pharmacy communications
Follow up on consultant visits and implement recommendations
Supervise Medication Aides/Techs during medication administration
Support audits and assist with quality improvement efforts

Requirements:Education
Graduate of accredited LPN School of Nursing.
Licenses/Certification
Current valid LPN license in the state employed.
Valid CPR certification.
Experience
Six months experience in a long-term care environment preferred.
