Our value-based care model helps us consistently deliver better patient experiences and outcomes.As a Lead Director, Pop Health - Specialty Care, you will play a critical role in supporting our Oak Street Health patients, providers and business by leading programs targeted at improving the Specialty care experience for patients and care teams and reducing specialty-care-related medical costs. The Lead Director will lead a team accountable for achieving specific objectives and key results, and will report directly to the VP, Specialty Care.The ideal candidate will have strong written and verbal communication skills, be extremely detail-oriented, comfortable taking ownership & following up, be self-directed, and enjoy working with a team in a fast-paced environment, while bringing their experience and innovative ideas.Core ResponsibilitiesDrive strategic problem solving, opportunity analyses & new idea generation for initiatives within the Specialty Care team at Oak Street, with emphasis on the Preferred Specialist Network and Referrals ManagementManage a team working towards clear objectives and key results, including but not limited to optimizing utilization of Preferred Networks and developing a best-in-class Provider Demographic DatasetOwn end-to-end strategic initiatives within the Specialty Care umbrella, from idea generation to Product building to field implementation, with support from said team membersDrive cross-functional stakeholder engagement across various teams at Oak Street Health (Product, Engineering, Epic, Field Leadership, Population Health Medical Directors) and CVS Health (Provider Data Management team, MinuteClinic leadership) to deliver on key results and realize synergies for the EnterpriseBuild deep relationships and advocacy for Preferred Networks Programming with Oak Street Health field leadership (business & clinical) through education, road-shows, clinic visits, and other creative approachesPartner with Epic product owners on development, design, functionality, and reporting needs as it relates to Specialty CareManage and own 3+ Vendor Partnership relationships in the Provider Data Management and Care Navigation spaces; continuously assess market landscape to ensure optimal Vendor Partners for Oak Street and CVS HealthIllicit feedback / input on Specialty Care initiatives from various field cohorts; partner with above stakeholders to iterate on workflows accordinglyEstablish communication cadence/content for regional leaders regarding awareness of, and engagement with, Specialty Care initiatives and establishing a shared commitment to performanceOther duties as assignedWhat are we looking for?We’re Looking For Motivated Individuals WithTrack record of effectively solving difficult problemsOrientation toward analytical problem solvingPractical, results-oriented mindset with unimpeachable integrity, accountability, and coachabilityAbility to manage multiple priorities effectively while maintaining a positive attitude5-7 years' experience at a health care consulting firm or similar analytical environmentStrong MS Excel and PowerPoint skillsAI and technological fluency demonstrated by incorporation in day-to-day workBachelor's degreeUS work authorizationWhat does being “Oaky” look like?Radiating positive energyAssuming good intentionsCreating an unmatched patient experienceDriving clinical excellenceTaking ownership and delivering resultsBeing relentlessly determinedWhy Oak Street Health?Oak Street Health is on a mission to “Rebuild healthcare as it should be'', providing personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
