The RN Case Manager will play a key role in care coordination, discharge planning, utilization review support, and interdisciplinary collaboration to ensure patients receive timely and appropriate care throughout hospitalization and transition planning.This role is ideal for candidates with recent acute care hospital case management experience who thrive in a fast-paced healthcare environment and possess strong communication and organizational skills.Key ResponsibilitiesCoordinate patient care and discharge planning within an acute care settingCollaborate with physicians, nursing staff, social workers, and interdisciplinary teamsPerform utilization review and care coordination activitiesAssess patient needs and develop effective discharge plansFacilitate transitions of care and arrange post-discharge servicesEducate patients and families regarding discharge plans and available resourcesMaintain accurate and timely documentation within hospital systemsEnsure compliance with hospital policies, regulatory standards, and case management protocolsParticipate in weekend rotation coverage as neededManage assigned caseloads with approximately a 1:20 patient ratioSupport patient throughput and continuity of care initiativesRequired QualificationsActive California Registered Nurse (RN) License requiredPending California license accepted; must be active prior to start dateMinimum 2 years of acute care hospital Case Management experience requiredRecent experience with hospital-based care coordination and discharge planning requiredStrong understanding of acute care case management workflowsExcellent communication, collaboration, and organizational skillsAbility to work effectively in a fast-paced hospital environmentFirst-time travelers accepted (clinical screening required)Flexibility to accommodate weekend scheduling needsPreferred QualificationsPrevious travel nursing experience in Case ManagementExperience in high-volume acute care facilitiesStrong interdisciplinary collaboration backgroundKnowledge of utilization review and discharge planning regulationsFor more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
