RN Home Health Nurse
Estimated Gross Weekly Pay: $2560$500 CONTRACT COMPLETION BONUSOpportunity:RN - 351011118 - Home Health/HospiceJob Details:• Work Monday through Friday from 8:00 AM to 4:30 PM (end time may vary)• See 5-6 patients per day with 25-30 points per week including routine and OASIS visits• Provide both home health and hospice visits for primarily geriatric population• 1 on-call week/weekend every 6 weeks (on-call days do not need to be consecutive)• Weekend work only required during on-call shifts• Must be available to work at least 1 major holiday and 1 minor holiday• Required certification: BLS (AHA)• Travel experience required along with critical thinking abilities• Must have skills in chronic disease management, patient/family education, NPWT, ostomy care, central line care, IV administration, and complex wound care• Hospice experience preferred including pain & symptom management, end-of-life care, empathy, and compassion• Daily 15-minute huddle calls, weekly IDT meetings at office, monthly all staff meetings• Work with patients having heart disease, chronic respiratory diseases, CHF, COPD, pneumonia, dementia/Alzheimer's, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacements, and post-surgical care• 1 day computer-based training followed by 2-5 days of field training/shadowing depending on experience• Dress code requires solid color scrubs with no patterns or logos unless company-issued, closed-toe non-slip shoes with socks or hosiery• Must clock in and out at facility• Use Cerner charting systemFacility:This medical center provides comprehensive home health and hospice services to the community, focusing primarily on geriatric care.
