Purpose of Job
This position is responsible for coordinating healthcare services, in partnership with patients/families and their providers, across the continuum of care in order to maximize health and functional outcomes, reduce the likelihood of complications from illness, promote individual accountability for health, assure appropriate utilization of resources, and promote patient self-care management.
Job Functions
Establishes collaborative and supportive relationships with physicians, staff, patients and interdisciplinary teams acting as a patient advocate and liaison to support the patient's effort to reach their health goals.
Acts as the primary clinical resource and support for patients
Develops, implements, updates and integrates patient care plans.
Completes patient intake, orientation and education ensuring continuity of patient care and resources.
Participates in Quality Improvement initiatives.
Perform Clinical Support functions as needed.
Performs other duties as assigned.
Minimum Education/Work Experience
Graduation from Medical Assisting program or School of Practical Nursing
Strong customer service experience
3+ years of experience in a primary care setting
Knowledge/Skills/Abilities
Ability to serve patients in a friendly and efficient manner
Ability to meet and maintain the necessary background checks as aligned with position functions
Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above
Ability to set priorities and meet deadlines within the context of workload variations and interruptions
Exceptional communication skills
Knowledge of basic computer functions and Microsoft Office applications
Ability to respond appropriately to unpredictable situations and diverse patient populations
Ability to provide exceptional customer service
Capable of making independent decisions with the use of good critical thinking and problem-solving skills
License/Certification
Current LPN Licensure or Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) (new grad LPN's and MA's will have 6 months to become licensed/certified in order to maintain employment)
Current Healthcare Provider CPR Certification
Preferred Qualifications
