Internal Medicine Nurse Practitioner

Solomon Medical is seeking a full-time Nurse Practitioner or Physician Assistant to join our growing post-acute care practice serving skilled nursing and rehabilitation facilities throughout Monterey County.This role is ideal for an APP who enjoys caring for medically complex patients, values autonomy, and wants to work in a collaborative, well-supported clinical environment. Providers work closely with the Medical Director, interdisciplinary facility teams, and a responsive operations team while caring for post-acute rehabilitation patients and long-term care residents.Post-acute medicine offers clinical variety, meaningful patient relationships, and the opportunity to stay engaged in complex care while maintaining a more flexible and sustainable schedule than many traditional healthcare settings.This is an onsite clinical role with strong support systems in place, including medical leadership, APP collaboration, dedicated medical coordinators, and organized workflows that help providers stay focused on patient care.For APPs interested in long-term growth, Solomon Medical also offers mentorship, professional development, and opportunities to take on expanded clinical responsibilities as the practice continues to grow.
