Develops and monitors treatment plans, manages complex/co-occurring conditions, and collaborates with multidisciplinary teams to ensure coordinated care. Engages diverse populations with culturally sensitive approaches and connects patients to community resources. Utilizes multiple care modalities (in-person, telehealth) and ensures timely, compliant documentation and HIPAA adherence. May supervise behavioral health staff.
