This position is designed for those who require immediate support to connect with vital community services and resources. The HAL will primarily focus on individuals at heightened risk of hospitalization, re-hospitalization, or suicide, especially those who do not qualify for other services during the assessment phase.Key ResponsibilitiesProvide timely support and care for individuals being discharged from hospitals, ideally co-locating for efficient access to referrals.Implement short-term, community-based interventions for up to 90 days following initial contact.Travel to meet individuals in the community as necessary.Maintain regular communication with hospital personnel to promote continuity of care and address referral workflows.Conduct follow-up calls and outreach to referred individuals as deemed appropriate.Attend monthly meetings and weekly clinical supervision sessions with the Director of Access & Urgent Care Services.Meet established productivity levels as agreed upon.Provide culturally sensitive services tailored to the diverse needs of each individual.Acquire comprehensive knowledge of all services offered by Compass Health, as well as other relevant agencies for referral purposes.Ensure meticulous documentation of client records in compliance with agency and state regulations.Collaborate effectively with multidisciplinary treatment teams when referring existing clients.Required SkillsProfound understanding of behavioral health challenges.Proficient in delivering culturally competent services.Exceptional communication and collaboration abilities.Capable of maintaining professional documentation and client records.Familiarity with community resources and human/social service agencies.QualificationsMust possess a Master’s degree in social work, psychology, or a related field.A minimum of three (3) years’ experience within the behavioral health sector is required.Valid MO driver’s license with agency-established minimum automobile coverage.A solid understanding of DSM-V is essential.Licensure or certification (PLPC, LPC, LMSW, LCSW, PLP, or LP) is preferred.Career Growth OpportunitiesThis position offers avenues for ongoing training and professional development, fostering skill enhancement in the behavioral health sector.Company Culture And ValuesAt Compass Health, we are committed to fostering a collaborative and culturally sensitive work environment that prioritizes the unique needs of individuals.
