In this role, you will provide expert clinical care across a broad spectrum of patient needs, from acute to chronic conditions, ensuring personalized treatment plans that promote health and wellness. Your expertise will help us uphold our commitment to exceptional patient-centered care in a supportive environment. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews. Diagnose and manage acute and chronic illnesses across diverse age groups, including pediatrics and geriatrics. Administer injections, perform suturing, IV infusions, catheterizations, and other clinical procedures with precision and sterile technique. Utilize EMR (Electronic Medical Record) systems such as Epic, eClinicalWorks, or Cerner for accurate documentation of patient encounters. Coordinate care plans involving medication administration, wound care, telehealth consultations, and follow-up assessments. Collaborate with multidisciplinary teams for case management, discharge planning, and utilization review. Provide health coaching and education on disease prevention, nutrition, family planning, and lifestyle modifications. Experience Valid Nurse Practitioner license with current certification to practice in the state. Proven experience in primary care settings with familiarity in outpatient clinics or medical offices. Critical care experience or hospital medicine background preferred; experience with emergency medicine or ICU is a plus. Skilled in performing clinical procedures such as suturing, phlebotomy, venipuncture, spinal taps, and dermal fillers. Strong understanding of HIPAA regulations and infection control practices to ensure patient confidentiality and safety.
