Job OverviewAs an Clinical Nurse Navigator — Oncology, you will serve as a dedicated clinical partner for individuals navigating the complexities of a cancer diagnosis. You will provide personalized education, comprehensive care coordination, and unwavering clinical support from the point of diagnosis through the survivorship journey.This role is designed for a highly skilled RN who balances clinical precision with deep empathy, ensuring that every patient receives equitable, evidence-based guidance in a fast-paced, technology-enabled environment.Key ResponsibilitiesCaseload Management: Oversee a full, active caseload, providing individualized navigation and clinical support tailored to each patient’s unique journey.Clinical Assessment: Conduct comprehensive oncology intake assessments to identify patient needs, personal goals, and immediate care priorities.Patient Education: Educate patients and their families on diagnoses, treatment pathways, and clinical options using NCCN-aligned and ONCC-aligned evidence-based guidance.Care Coordination: Facilitate seamless communication between patients, multidisciplinary treating providers, and internal clinical teams to streamline care delivery.Barrier Mitigation: Identify and resolve psychosocial, financial, or logistical barriers to care, connecting patients with essential support resources.Clinical Documentation: Maintain meticulous and timely records of all interactions and clinical activities within the care management platform.Quality Assurance: Participate in call auditing processes and integrate feedback to ensure the highest standards of care and protocol adherence.Multidisciplinary Collaboration: Partner closely with Lead Navigators, Social Workers, Medical Records Specialists, and Care Advocates to provide holistic support.Required QualificationsEducation: Bachelor of Science in Nursing (BSN).Licensure: Active RN license in a compact state with the unencumbered ability to obtain licensure in all 50 states.Experience: Minimum of 3 years of dedicated oncology nursing experience.Certification: Active ONCC certification (OCN); must be maintained throughout employment.Bilingual Proficiency: Fluent in English and Spanish, with the proven ability to conduct complex clinical conversations in both languages.Tech Savvy: Proficiency in Microsoft Office Suite (Outlook, Teams, etc.) and familiarity with NCCN guidelines Hybrid Capability: Must have a private, secure home workspace for remote days, free from distractions or other obligations during working hours.
