You'll follow an individualized care plan under the direction of a registered nurse or therapist, helping patients with daily tasks while preserving their dignity and independence.Key Responsibilities:Personal Care Assistance: Help patients with bathing, grooming, dressing, and toiletingMobility Support: Assist with transfers, walking, exercises, and fall preventionMonitor Vital Signs: Check and document temperature, pulse, and respiration during each visitMeal Preparation: Assist with meals and feeding based on the patient's dietary planMedication Reminders: Prompt patients to take prescribed medications (no administration required)Light Housekeeping: Change bed linens, keep the environment tidy and safeObserve & Report: Monitor patient condition and communicate changes to clinical staffFollow Care Plan: Deliver care based on each patient's personalized home health care planRequirements:Current CHHA Certification through the California Department of Public HealthReliable Transportation You'll be traveling to patients' homes across Orange CountyCPR Certification Current and validStrong Communication Skills Fluent in spoken and written EnglishEmpathy & Professionalism Compassionate, respectful, and dedicated to patient-centered careHome Health Experience Preferred, but not requiredReady to Make a Difference?If you're passionate about helping others live safely and independently at home, Remedy Home Health is the place for you.
