CLINICAL PHARMACIST – COMMUNITY PHARMACY
The Hospital is currently seeking a full-time Clinical Pharmacist to join our dedicated healthcare team in our Community Pharmacy. The Clinical Pharmacist delivers direct, patient-centered care while supervising and facilitating the medication dispensing process. This role works closely with our clinical staff to provide high-quality pharmacy services and offers opportunities to be involved in clinic-based ambulatory care services.

Providing patient care in a rural practice setting to a diverse population
Collaboration with hospital and clinic-based providers and the interdisciplinary team
Practicing through collaborative practice agreements with clinic providers
Opportunities to precept pharmacy students
Upholding Wisconsin Pharmacy Quality Collaborative (WPQC) best practices to ensure high-quality patient care
Opportunities to be involved in clinic-based services like Chronic Care Management and Anticoagulation/INR Clinic
Opportunities for participation in hospital-wide committees and taskforces
Education Required:
Graduation from an accredited school of pharmacy
Credential/Licensure Required:
Valid, unrestricted Wisconsin pharmacist license
Basic Life Support within 3 months of hire
Certified Immunization Provider within 3 months of hire
Skills Required:
Ability to interact with and work around people and to work effectively in a team environment.
Ability to make decisions, act, and maintain self-control under stressful situations and workload pressures.
Ability to practice pharmacy with integrity
Excellent written/verbal communication skills and ability to listen empathetically.
Ability to gather, learn, retain, recall, and communicate complex clinical and technical information.
Ability to function with minimal supervision, act independently, and demonstrate initiative
Ability to prioritize work appropriately and focus on multiple tasks simultaneously.
Proficient to expert computer and application skills as applicable to role.
Comfortable accepting responsibility for small to large scale projects involving multiple resources and spanning many months from start to finish.
Ability to work on-site
Regular, reliable attendance.
Work Schedule:
Pharmacy Hours: Mon-Fri 8:30 AM - 5 PM
Shift: Varies
