Outpatient Therapist

Job Summary Responsible for assisting in the overall coordination and delivery of social work and mental health services as it relates to Memorial Hospital and Health Care Center. Demonstrates knowledge of needs of patient population. Individual will provide social work and mental health assistance in accordance with the mission and core values of Memorial Hospital and Health Care Center. Provides telehealth assessment of patients referred by identified referral sources on an urgent and emergent basis. Assists patients in completing forms. Makes referrals to other agencies as needed. Communicates on behalf of the patient with family members or significant other in emergency situations. Communicates with providers, hospital personnel, County Judges and law enforcement as needed. Work to facilitate and/or provide continuity of care for clients needing social work and/or behavioral health assessments and services with identified and referred clients for Memorial Hospital and Health Care Center. Duties may include performing a psychosocial assessment on each client and the participation in the development of each client's care plan. Participates in staffing of identifies patients. Completes regulatory and other reporting measures and tracking duties as assigned. Qualifications Education: Master's Degree in Social Work or equivalent human services field required (Psychology, Sociology, Mental Health Counseling).Must have Masters degree. Must have License from Indiana Professional Licensing Agency or have education plan for obtaining Licensure, as approved by the Director of Behavioral Health Services. Maintains continuing education requirements according to the appropriate Licensure Board. Training: Requires 6-10 weeks of orientation including a minimum of 40 hours shadowing another experienced practitioner with ongoing Social Work supervision. Experience: Two to three years social work and behavioral health experience in a health or mental health care setting preferred. Job Knowledge, Skills, and Abilities: Demonstrates an understanding of social work and mental health care. Able to use independent judgment and work alone as frequent independent decisions are required and can have impact on policy, cost, and/or patient care. Excellent communication and written skills as well as a sound and healthy emotional well-being. Position requires a high degree of concentration, analytical and/or creative thinking over extended period of time. Ability to interact effectively with all levels of the organization. Ability to communicate departmental duties and/or roles, community resources, and legal information to others in a clear, understandable manner. Computer skills are needed for electronic documentation purposes. Demonstrates ability to relate to clients and their families and an ability to function in a multi-disciplinary setting. Must have excellent communication skills and ability to relate and communicate to mentally ill clients, their family members, friends, and hospital staff including providers. Familiarity with organizational, functions, policies and procedures as they relate to the Behavioral Health and Social Work Services Department. Knowledge of state and federal guidelines including Indiana State Department of Health and The Joint Commission. Participation in Quality Counsel process. Completion of competency skills checklist within 60 days. Knowledge of Code situations, Infection Control, and Standard Precautions as they apply to the Division of Patient Services. Performance Requirements Assists in the delivery of social work services as it relates to Memorial Hospital and Health Care Center. Duties may vary day to day in the sense of working with diverse populations. Responds to urgent and emergent requests from patients, providers, and other referral sources and provides patient assessment and recommends course of action. This assessment will be via use of telehealth technology. Assists patients with completing forms, complete charting and other documentation as appropriate. Makes follow-up phone contacts to facilitate and assist with patient needs for follow-up care and other services. Complete regulatory reporting measures and internal tracking measures appropriately. Provides at least an initial assessment as requested to patients referred to this program who may not be seen by another provider, unless circumstances warrant not doing so. Works to meet the referral request for those patients determined to need those services. Must understand and demonstrate an ability to provide recommendations and resources to the patients in need of services. Maintains own caseload and is responsible for timely disposition of patients. Involved in scheduling of patients, intake process and telephone intake. Provides individual, group and family therapy and documents patient's level of interaction and behavior for every encounter/scheduled appointment. Participates in overall quality improvement plan and departmental meetings/updates. Keeps abreast of departmental Policies and Procedures, as well as Hospital wide Policies and Procedures. Completes all necessary documentation for patients in a timely manner, complying with all Federal and State mandates. Keeps current of pertinent State and Federal guidelines and law. Keeps current with available community resources and how to access them. Assignments given by the Behavioral Health and Social Work Services Leadership as well as the Psychiatric Mental Health Nurse Practitioner assigned to this program, and individuals who may be referring patients to this program. Duties may vary day to day in the sense of working with diverse populations. Primary job duties will be evaluating and assisting patients with social service and mental health needs to access resources and treatment by means of telehealth technology. Physical Demands: Walks, sits, and stands intermittently. Lifting weights less than ten pounds. Visual, auditory, and speech acuity essential for talking/communication required with a diverse population. Works in office areas and departments which are well-lighted, heated, and ventilated. May come in contact with hazardous health conditions; however, if proper procedure is followed no harm should come to employee. Special Demands: Maintains composure in stressful situations, continually providing emotional support for patients and families. Requires a high degree of concentration in order to make accurate decisions. Willingness and ability to adapt schedule to meet the demands of the department. Must be able to communicate verbally, visually, and through written instructions on progress notes. Must be able to work cooperatively with a multitude of departments within communities and Memorial Hospital was well as with a multitude of community resources. Must demonstrate sensitivity to others. Must have sound crisis intervention skills, so as to adequately assess a situation, especially those which are life threatening. Responsible for decisions that could seriously impact patient care, policy or cost. Maintains confidentiality. Must adhere to the NASW Code of Ethics and all MHHCC policies and procedures.
