Principal, Care Integration & Clinical Program Delivery
Become a part of our caring community We are looking for a clinician leader to design clinical programs that improve outcomes for senior and high-risk patients. You will report to the Director of Physician Strategy. You will combine clinical expertise with program development and population health strategy.You will spend most of your time building and improving care programs across the organization. You will also maintain a small amount of clinical practice time to stay connected to patients and care teams.This is a senior-level individual contributor role. You will work closely with clinical, operational, and cross-functional partners.Use your skills to make an impact What You'll DoDesign clinical programs for high-risk and senior patient populationsTranslate population health goals into clear workflows, care plans, and processesDevelop standardized approaches for care management, transitions of care, and chronic condition supportPartner with teams across operations, analytics, product, compliance, and educationCreate program materials, documentation standards, and training contentIdentify gaps in care delivery and redesign workflows to improve outcomes and efficiencyEnsure education teams understand programs and deliver them.Use insights and data to improve program performanceManage projects from planning through implementation and ongoing improvementWhat Success IsPrograms are clear, scalable, and used across marketsCare teams understand workflows and can follow them easilyProgram changes improve patient outcomes, coordination, or resource useTeams are aligned and able to implementYou take ownership of work and improve resultsRequired QualificationsLicensed Nurse Practitioner (NP) or Physician Assistant (PA)Active, unrestricted U.S. license in consideration of your work locationWork in a hybrid model (approximately 90% remote, 10% in clinic weekly)8+ years of experience across clinical care, operations, or program deliveryExperience caring for senior, geriatric, or complex patient populationsExperience designing or leading clinical programs, population health programs, or care modelsTranslate clinical goals into workflows, processes, and measurable outcomesExperience in value-based care, Medicare Advantage, or risk-based modelsBackground in care management, transitions of care, or population health programsExperience working with interdisciplinary teams (nursing, pharmacy, social work)Familiarity with EMRs and care management platforms (e.g., Athena, Salesforce Health Cloud)Experience supporting training, or program rolloutMain SkillsClinical program developmentPopulation health and care model designWorkflow design and process improvementCross-functional collaborationChange management and implementationWork EnvironmentHybrid role in consideration of a Conviva or CenterWell marketApproximately 90% remote program work and 10% in-clinic care (half-day per week)The company designs limited clinical time to maintain connection to patients and care delivery.No direct reports; operate as a senior individual contributorTravel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.Scheduled Weekly Hours40Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $150,000 - $206,300 per year This job is eligible for a bonus incentive plan.
