Licensed Vocational Nurse (LVN) for Home Health

Physicians Choice Home Health is hiring a compassionate and skilled full-time or part-time Licensed Vocational Nurse (LVN) to provide high-quality home health visits to our patients. Obran Health is building the largest worker-owned, home-based healthcare organization in the U.S. We're giving healthcare professionals real ownership, a voice in decision-making, and a share in the success they help create.What You'll DoProvide in-home nursing visits under the supervision of a Registered Nurse (RN), following the established plan of careDeliver treatments, assist with procedures, and use appropriate techniques when preparing equipment and supporting patient careAssess patient condition, provide education to patients and caregivers, and reinforce self-care and treatment plansDocument visits, care provided, and patient status in a timely and accurate mannerCommunicate changes in patient condition and coordinate care needs with the RN and interdisciplinary teamSupport care coordination by identifying needs for additional services (PT, OT, ST, MSW, HHA, etc.)Use medical equipment and supplies safely and efficiently in the home settingWhy Physicians Choice?
