In addition, therapists will provide behavioral health consultation services to the Primary Care Teams within Pediatric and Adult outpatient clinics:POSITION QUALIFICATIONSMaster's in Social WorkLCSW (California license)California Driver's LicenseTwo years' experience providing clinical services in the mental health fieldBilingual English/Spanish preferredDUTIES AND RESPONSIBILITIESConsultations about clientsInitial mental health evaluations resulting in realistic treatment plansAssessing clients using standardized assessment/screening tools, measures, etc.Provide brief, solution focused therapy;Provide group psychoeducation/treatment related to chronic medical conditions;Provide information and referral services;Care coordinationCrisis assessments/triage with appropriate interventionsProvision of other services related to wellnessDocumentation of services provided in accordance with requirementsOther related activities designed to provide integrated health care servicesIndividual psychotherapy with children, adults and familiesDocumentation and scheduling in electronic health recordProvide psychotherapy to children and adults; • other duties as assignedSalary Description
