Genesis provides a wide range of services dedicated to improving the health of our communities in the Pee Dee and Low Country regions of South Carolina.POSITION SUMMARYThe LPN is responsible for coordination of care between patients, medical providers, and administrative clinical staff. The LPN's primary responsibilities are providing support for the effective delivery of health care services in the clinic, direct patient care and provider support.PRIMARY ACCOUNTABILITIESAchieve ResultsMaintains a professional image and exhibits excellent customer relations to patients and visitors, as well as vendors, contractors, and related resources.Ensures services in a timely manner to patients.Provide support for physicians and other clinical staff, maintaining an acceptable level of productivity and consistency with all tasks as assigned.Ability to work well with others and be a team player.Ability to take initiative and handle various tasks simultaneously while working efficiently, effectively, and independently under minimal supervision.Operational Excellence Ensure and uphold the confidentially requirements of all patient records and manage all daily task and activities consistent with HIPAA, state and federal laws and regulations, as well as the clinic's policies and regulations regarding confidentiality and security.Relationships Develop and ensure effective, positive relationships between and among the clinic staff.Professionalism Ensure all actions, job performance, personal conduct and communications always represent the organization in a highly professional manner.Uphold and ensure compliance with and attention to all corporate policies and procedures, as well as the mission and values of the organization.PRIMARY TASKS & DUTIESServe as an advocate and health educator to patients, families, and the community.Promote health to prevent disease and help patients cope with illness.Prepare patients for and assist with examinations, administer injections to all age groups, assist with lab draws, monitor vital signs, maintain records on the Electronic Medical Record.Assist with procedures such as pap smears, breast exams and complete plan of care and referrals after visit if necessary.Assist with medication refills, medical record request and medication authorizations.Facilitate clinic flow and room turnoverReviews each patient chart before appointments compiling their disease process and diagnosis noted while updating all testing, screening and vaccinations associated with their diagnosis.Schedules follow up appointments for the patients and ensure understanding of these dates and times.Review daily abnormal lab values assigned by the Provider and make appropriate calls, to complete orders related to the lab values. Follow up of these orders are recorded in the EMR and confirmed with ordering provider.Responsible for taking initial and interim history, as well as vital signs when triage staff unavailable.Checks equipment and supplies, restocking as necessary and checks emergency kits as directed.Responsible for maintaining required education.Responds to phone calls in a respectful and timely manner.Serve as relief coverage for all qualified positions, as needed.Must be able to commute to other Genesis Healthcare, Inc. locations, as neededAny other duties as assigned ESSENTIAL FUNCTIONS/KEY COMPETENCIESFunction in a multi-tasking, multiple priorities environment, while maintaining accuracy and attention to detail.Communicate with a diverse employee population.Utilize a wide variety of office technology and tools to collect, enter, process, file patient records and documentation Expected hours: 36 - 38 per week Benefits:401(k)401(k) matchingDental insuranceHealth insuranceLife insurancePaid time offRetirement planVision insuranceMedical Specialty: Primary CarePhysical Setting: ClinicWork Location: In person RequirementsEducationPossession of diploma in Nursing. Must be able to speak, read, write, and understand English.Must have an unrestricted active license. in the State of South Carolina.Professional Four years of professional experience in a Health Care Organization, working directly with Medical Providers and Patients preferred.Communication, customer service or working with the public, preferably in a medical care facility.Knowledge of medical terminology preferred.Physical/Environmental Ability to interact with computer screen for up to six hours at a time (visual acuity required).Must have manual dexterity for use of keyboard. Ability to stand for periods of up to two hours. Ability to communicate via phone, mail and in person to resolve patient issues.Capacity to function in a sometimes stressful, multi-tasking environment
