RN FIELD CASE MANAGER | SUMMARY AND HIGHLIGHTS: The RN Field Case Manager is responsible for performing comprehensive assessments and coordinating individualized care plans for high-risk members to ensure optimal clinical outcomes. This role involves managing care transitions, providing specialized home health interventions (such as wound care and IVs), and serving as a patient advocate to bridge the gap between medical providers and social support services. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates RN FIELD CASE MANAGER | RESPONSIBILITIES: Comprehensive Assessments: Perform detailed assessments of high-risk members to identify medical and non-medical needs. Care Coordination: Collaborate with primary care providers, social workers, and discharge planners to implement and manage individualized care plans. Patient Advocacy: Act as a liaison for the member, ensuring their privacy is protected and they are transitioned to the appropriate level of care. Clinical Management: Continuously reassess members to ensure cost-efficient and medically appropriate care, while putting support systems (like housing or transportation) in place to ensure compliance. Documentation: Maintain precise clinical, functional, and fiscal records throughout the management process. RN FIELD CASE MANAGER | REQUIREMENTS: Education: Associate’s or Bachelor’s Degree in Nursing (or a related field). Attendance: Commitment to mandatory attendance during the first 90 days. Licensure: Must hold a current, unrestricted Registered Nurse (RN) license for Alabama. Ability to analyze complex medical data and make rational, patient-centered decisions. Commitment: Availability for the M-F schedule and the required every-other-weekend on-call rotation. Preferred Qualifications Home Health Experience: Prior experience with IVs, wound vacs, and OASIS documentation is highly desired. Specialized Clinical Skills: Experience with ostomies, Foley catheters, or chest tubes. Certification: Case Management certification is a plus. Take the Next Step!
