About The RoleThe case manager is responsible for the implementation and management of the resident Individual Treatment Plan (ITP). Our case manager is responible for: Coordinating, monitoring and evaluating educational services Scheduling and monitoring community activities Facilitating Functional Independence Measure (FIM) communicationsRequirementsMust have at least a Bachelor's DegreeMust be Med-Tech certifiedMust have 40 hours of training completedMust complete MANDT trainingAbout The CompanyJumoke, Inc. is a human services agency that provides residential and independent living placement to foster care and other at risk youth in Baltimore, Maryland.
