Position: Intake Coordinator Classification: Non-Exempt Supervised by: Program Manager Supervises: N/A Home Start’s vision is for every child to have a safe, stable, and nurturing home. Home Start, Inc. operates programs designed to prevent homelessness, interrupt the cycle of homelessness, and promote self-sufficiency. With rising rates of homelessness in San Diego exacerbated by the pandemic, our programs are designed to ensure homelessness is rare, brief and non-recurring. Job Summary: The Intake Coordinator is the first point of contact for walk-ins and referrals experiencing homelessness or at-risk of homelessness outside of HMIS. The Intake Coordinator manages the housing department’s intake phone line, email, various program waitlists, and assists clients with navigating both internal and external programs and resources. This role provides light touch case management services and connects clients to appropriate services and assistance. Duties & Responsibilities: Manages housing phone line and emails, responds to online referral inquiries, and oversees and coordinates office hours. Respond to all inquiries about programs, screen for services/assistance, update and maintain program waitlists, and help clients navigate internal and external resources. Conducts immediate needs assessments and provides crisis stabilization. Coordinate and refer services with other social service agencies to provide immediate mental health, medical, dental care, and/or rehabilitation services. Leverage resources available within the community including faith-based organizations as appropriate to the situation. Maintain case notes and required file documentation. Provide person-centered system navigation aligned with evidence-based approaches like housing first model, progressive engagement, crisis stabilization, harm reduction, and trauma-informed care. Attends staff meetings, case conferences, trainings, and supervision meetings. Works collaboratively with co-workers to assure smooth and seamless delivery of comprehensive services to clients. Qualifications: Must be vaccinated from Covid-19. Undergraduate degree in Social Services or related field. Experience may waive education requirements. Minimum of 1 year working or volunteering with underserved populations. Lived experience is a plus for this position. Must submit to a background check via Live Scan (with ongoing alerts), as required by specific programs, funding requirements and licensing regulations. Must have a valid driver’s license and a satisfactory driving record. Must demonstrate ability to be culturally sensitive and respect diversity. Ability to work in a fast-paced environment with exceptional problem-solving skills. Physical Demands: Reading and typing on a computer frequently. Sitting for long periods of time at required meetings or in supervision. Valid Driver’s license and own vehicle to meet with clients and/or attend agency or community meetings. Ability to be flexible and work in various locations (including weekends if needed).
