**Job Title: Home Health Physical Therapist****Location:** St. Paul, MN and surrounding areas**Job Description:**We are seeking a skilled Home Health Physical Therapist to provide patient care in home settings. This role demands expertise in home health and OASIS documentation.**Responsibilities:**• Deliver physical therapy services to home health patients.• Complete OASIS documentation on the same day.• Manage a caseload of 20• 25 cases weekly.• Attend weekly in• person office meetings.• Use Homecare Homebase for documentation.• Be flexible to float as needed.**Qualifications:**• Valid PT License and BLS certification.• 2+ years of home health experience.• Proficient in OASIS E documentation (post• January 2023).• No more than a 1• year gap in relevant experience.**Additional Information:**• 13• week contract, 40 hours/week.• No on• call or overtime.• Mileage reimbursement.• Flexible hours, branches open 8 am• 5 pm.**Requirements:**• Reside within 30 minutes of the territory or secure nearby housing.• Pre• approval required for time off.**Orientation:**
