We operate two Community Residential Services (CRS) facilities (North and NE Minneapolis).We are seeking for part-time Registered Nurse to conduct assessments, evaluations, assist in preparing care plans, perform medication set-ups, supervise HHA's and CNA's and assist with caregiver training.RESPONSIBILITIESConduct initial assessments, admission, case management, and follow-up visitsPerform ongoing care and case management for each client including re-evaluations of client needs, care and progress on a regular basisDevelop a plan of care based on physician orders and initial client assessment - revise and update based on ongoing assessmentsFacilitate appropriate care including referrals to additional services (Occupational Therapy, Physical Therapy)Perform medication set-upsCoordinate, oversee, and supervise on-site CNA / PCA / HHA visits and ensure plan of care is being followedReport changes in client condition to physicians and Director of NursingMaintain RN license annuallyREQUIREMENTSCurrent Minnesota RN licenseApplicants must be able to pass Minnesota Net Study background study, Tuberculosis Screening, and general background investigationPrevious experience with seniors and client assessmentsExcellent customer relations and interpersonal skillsExcellent verbal/written communication skillsValid driver's license and insured personal vehiclePREFERREDExperience in Corporate foster care and/or CRSSupervisory experienceWork RemotelyNoJob Type: Part-timePay: $45.00 - $55.00 per hourSchedule:On call and as needed (estimate 10 hours a week)Education:Associate (Preferred)Experience:Nursing: 1 year (Preferred)
