They currently boast a team of more than 30 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, and transitional care.POSITION DESCRIPTION: The physician will serve as the primary care provider for a dedicated panel of geriatric patients.Patients are located in long-term care facilities (ALFs, ILFs, SNFs) and private homes. Physicians are given a reasonable geographic area in which all patients are located (to ensure minimal daily drive time) and there is flexibility in regard to whether house call patients are included in the panel.Providers see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs.
