Licensed Clinical Social Worker (LCSW) Location: Prescott, AZ Agency: Jackson Therapy Partners Pay: $1,873 to $2,073 per week Shift Information: Days - 5 days x 8 hours Start Date: ASAP About the Position Make a meaningful impact as a Licensed Clinical Social Worker in Prescott, AZ. We seek an experienced LCSW or LMSW with hospital and acute care experience to advocate for patients, coordinate discharge planning, complete social work assessments, and address behavioral health, substance use, homelessness, and social determinants of health across inpatient, emergency department, ICU, and maternal/child settings. Minimum Qualifications Master’s degree in social work (MSW) from an accredited program. Current state license in good standing (LCSW or LMSW; LCSW preferred). Minimum one year of hospital setting experience required. One full year of hospital experience is mandatory. Minimum IMSW with at least two years of supervised clinical experience preferred. Experience with Cerner electronic medical record system required. Experience conducting inpatient and emergency department social work assessments, discharge planning, Title 36 petitions, DCS/APS reporting, and working with maternal-child populations preferred. Ability to manage caseloads typically ranging 12–16 patients per day. BLS is not required. Assignment Details Facility type: Skilled Nursing Facility with responsibilities across inpatient, ED, ICU, and family birthing center settings. Shift: Days. Shift hours: 0800–1600 (typical inpatient hours 0800–1630; ED hours may vary, e.g., 1000–2000). Guaranteed hours: 40 hours per week. Orientation: Department-specific onboarding with Social Work Manager, approximately 12 orientation hours plus 1–2 week onboarding period. Weekend requirement: Every other week rotation; weekends may occur approximately every 3–4 weeks. Charting system: Cerner. Must not have been employed (FT, PT, or PRN) with facilities associated with CommonSpirit, CHI, or Dignity within the past year. Radius rule: 50 miles.
