Licensed Clinical Social Worker (LCSW) Location: Prescott, AZ Agency: Jackson Therapy Partners Pay: $1,873 to $2,073 per week Shift Information: Days - 5 days x 8 hours Start Date: ASAP About the Position Make a meaningful impact as a Licensed Clinical Social Worker in Prescott, AZ. We are seeking a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with hospital-based experience to provide comprehensive social work services across inpatient settings, emergency department, family birthing center, and ICU. Combine clinical assessment, discharge planning, and community resource navigation while supporting diverse patient populations dealing with mental health, substance use, homelessness, and social determinants of health. Minimum Qualifications Master’s degree in social work (MSW) from an accredited program. Licensed at minimum as an LMSW; LCSW preferred. State license in good standing required. Minimum one full year of hospital setting experience is required. Preference given to candidates with at least two years supervised clinical experience. Experience with Cerner electronic medical record required. Experience performing social work and mental health assessments, discharge planning, Title 36 petitions, and reporting to DCS/APS as needed. Ability to manage caseloads typically ranging 12–16 consults per day. Must meet client-specific eligibility: not employed (FT, PT, or PRN) by any CommonSpirit-associated facility, including CHI or Dignity, within the past year; must be within the client’s 50-mile radius rule. BLS is not required. Assignment Details Facility type: Skilled Nursing Facility and hospital-based coverage across inpatient, ED, family birthing center, and ICU. Shift: Days. Shift hours: 0800–1600 (typical inpatient hours documented as 0800–1630; ED coverage may require 1000–2000). Weekly hours: 40 hours per week, guaranteed. Orientation: Department-specific onboarding with SW Manager; approximately 12 orientation hours and 1–2 week onboarding. Caseload: Varies, approximately 12–16 patients/consults per day. Weekend requirement: Every other weekend (EOW) with actual frequency approximately every 3–4 weeks at most. Float requirement: May be required to cover multiple hospital departments; candidates should expect to work across units as needed. Charting system: Cerner.
