STG International is assisting the Veterans Admninistration with recruiting a full-time Physician for the Geriatric Evaluation and Management (GEM) clinic. This is a Federal staff position with their benefits, pension, etc. They will accept any Board Certification for this position.GEM is a specialized program by an interdisciplinary team of health care professionals for a targeted group of predominantly older patients and others with medical complexity.Inclusionary patient criteria:Multiple medical, functional, or psychosocial problems who could benefit from an interdisciplinary team approach orOne or more geriatric syndromes ie; dementia, delirium, functional decline, urinary incontinence, polypharmacy, elder abuse, unsteady gait or falls, malnutrition or depression.Required Skills:The goal of the GEM program is to identify, assess, and address, in an interdisciplinary manner, the biopsychosocial status of severely disabled and frail older persons at risk for further decline and institutional placement, with the intention of optimizing their health, function, and ability to live with the greatest degree of independence suitable for their situation(s).
