Under general supervision of the Assistant Director of Nursing, the qualified candidate will be a registered nurse who will support the clinician in the management of patients and families living with diabetes, and act as a health care consultant by providing information on the management of diabetes for patients of all ages from infancy to geriatrics. S/he shall also support the mission, vision and values of Cooperative Health, and work towards realizing its objectives through support of teams within the department.Principal Accountabilities/Responsibilities:Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injectionDevelop and implement educational programs for diabetic staff, patients, and familiesAdvise patients as to long term complicationsProvide clinical resource and consultation to staff and clinicians.Assist patients in monitoring blood glucose for the purpose of medication control and adjustment for both insulin and non-insulin dependent diabeticsAssess, monitor, and implement treatment plans for patients at various stages of the disease. The focus is on prevention of complications and management of stable diabetes.Document assessments, interventions, and plans including recommendations and goals, progress notes, and discharge summaries.Perform other related duties as requiredEducation & Experience:Bachelor's degree from an accredited nursing school preferredCurrently licensed to practice as a Registered Nurse in the state of South CarolinaCurrent BLS certificationCertified Diabetes Educator or the ability to obtain certificate within two years of employmentProven leadership abilities, time management and organizational skills, and have knowledge of policies, procedures, quality assurance and compliancePrevious experience with electronic medical records, with knowledge of statistics, data collection and Health Informatics preferredCompetenciesAbility to embody the mission and vision of Cooperative Health.Excellent written and verbal communication and problem solving skills.Ability to communicate with people from a variety of socioeconomic and cultural backgrounds.Ability to prioritize, organize and carry out work assignments independently and efficiently.Ability to maintain appropriate degree of confidentiality.Proficient in use of computer programs including Word, Excel, PowerPointCritical thinking, strong judgment, problem-solving, and multi-task during day-to-day work challengesEffective realization of performance objectives and previous experience in performance-based evaluations.Effective interpersonal and written/oral communication skills, positive attitude and an ability to motivate staffFamiliar with data collection required to meet PCMH, UDS, & Hedis quality standardsTime management, ability to organize and prioritize tasks, flexible, detailed oriented, innovative, and dependablePhysical DemandsProlonged periods sitting or standingMust be able to lift up to 25 pounds.Be able to sit, stand, stoop, squat for extended periods of time throughout the day.Standing or walking for extended periods throughout the day.Company Conformance StatementIn the performance of respective job assignments, all employees are required to conform with Cooperative Health's:Board approved policies and procedures;Confidentiality and professional provisions;Compliance program; andStandards of conduct.Cooperative Health provides equal employment opportunities (EEO) to all employees and applicants for employment without regard to race, color, religion, sex, national origin, age, disability or genetics.
