Field Nurse PractitionerLocations: Boise, ID; Palm Springs, Bakersfield, Fresno, CA; Middletown, New Haven, Litchfield, Willimantic, Rockville, New London, CTMolina Healthcare is seeking experienced Nurse Practitioners to provide in-home and community-based care, focusing on preventive services and health screenings. Work environments include patient homes, nursing facilities, and pop-up clinics.Key Responsibilities:Conduct comprehensive medical assessmentsProvide care coordination and treatment planningPerform face-to-face visits (home, SNFs, community sites)Use Epic EMR in the fieldTravel within a 60-mile radius; up to 120 miles occasionallyAttend mandatory 5-week trainingRequirements:Must be comfortable working autonomously in non-traditional settingsAble to manage diverse patient populationsStrong documentation and diagnostic skillsSchedule: 45 minutes per visit6 9 visits/day depending on shiftNo follow-ups or ongoing careJoin a mission-driven team delivering care where it s needed most.
