The CentraCare Senior Transitions team brings comprehensive, patient-centered care directly to individuals living in assisted living, memory care, long-term care, and transitional care settings — meeting patients where they are, in every sense of the phrase.SummaryOur interdisciplinary team works closely with patients, families, and fellow healthcare providers to deliver care that is preventative, proactive, and tailored to each person's unique needs. Together, we manage chronic conditions, address social determinants of health, support care transitions to and from inpatient settings — all within an integrated, collaborative model.As a provider on this team, you'll travel to assisted living, memory care, and long-term care communities across Central Minnesota, supported by a dedicated team of clinical operations and administrative colleagues working remotely alongside you. It's a role that offers autonomy, variety, and the deep satisfaction of building meaningful relationships with some of the patients who need it most.Provide on-site primary care to patients located in the long term care facilities in Melrose, Sauk Centre, Long Prairie, and assisted living in Long Prairie.Manage a panel of approximately 130 to 150 patients across five buildingsClinical support from Clinical Assistants and Care Coordination through LPNs, RNsAccess to diagnostics such as labs and x-rays as neededCollaborate with community partners within home care, hospice, and other resources as appropriateScheduleFull-time | 72 HoursDays Monday-Friday | 8-hour shifts | 7:30am – 5pmFlexibility to design and manage your schedule Call: 1:4 Weeks coverage from home overnight, weekends, and holidays for seven days approximately every four weeksPay and BenefitsSalary range: $109,990.40 - $139,880 per yearPay may be determined by factors such as education, experience, skills, knowledge, location, and internal equitySalary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rateMay be eligible for additional pay through bonuses, quality incentives, or production-based (RVU) compensation.Benefits: Medical, dental, time off, retirement, employee discounts and more!QualificationsRequired:Graduate of an accredited Nurse Practitioner Program or Physician Assistant ProgramBoard Certified/Board Prepared through AANP, ANCCLicensed or ability to obtain licensure through the Board of NursingAbility to obtain and /or maintain DEA certification for MinnesotaProven ability to communicate effectively with strong verbal skillsExcellent interpersonal and customer service skillsMust have a valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is requiredPreferred:2+ years of clinical experience as a Nurse PractitionerDepartment InformationSenior Transitions Primary Care
