Parent Coach

The Parent Coach, provides intensive case management, skill building, parent education, and support services to identified parents in the home or other natural environment Coordinates services and resources needed by the families to support their development and enable them to remain and function optimally in the community Provides support to the client and family in problem solving and decision making and provides information needed to assist families in advocating for their child and in developing an increased understanding of their child’s disability and parenting needs. Works with the Program Manager to develop effective and measurableIndividualized Service Plans (ISPs) at least once annually for parents/family receiving services and works to ensure supports and interventions are provided as stated in the plan. Ensures health and safety needs of consumers and parents are addressed as needed and monitored at least quarterly. Maintains a good working relationship with both internal and external referral sources and provides community education or training as needed. Promotes positive communication and collaboration among team members and other community resources needed to provide quality services. Conducts self in a professional manner at all times. Ability to establish and maintain good rapport with clients and families. Demonstrated ability to advocate for clients, and to develop and maintain working relationships within the agency and community and work as part of a multi-disciplinary treatment team Skills: - Strong communication skills, both verbal and written - Excellent listening skills and empathy - Knowledge of counseling/mentoring techniques and principles - Ability to effectively manage conflicts and diffuse tense situations - Strong organizational skills and attention to detail - Ability to work independently as well as part of a team - Non-judgmental attitude and ability to create a safe, supportive environment Note: This job description is not intended to be all-inclusive.
