Our Culture is built from our Core Values of Inclusivity, Collaboration, Accountability, Respect, and Empowerment (iC.A.R.E.)About the Role:The Case Manager manages an average caseload of 18 to 20 families at risk of homelessness or recently housed. TheCase Manager will support the housing first model with a trauma-informed care approach that will maximize thelikelihood of client success and long-term self-sufficiency. The position requires a self-starter as the responsibilitiesinclude: providing ongoing, community-based services, identifying, prioritizing, and addressing immediate needs,developing money management skills, supporting gainful employment search, coordinating training, andeducational goals, and address mental health and substance abuse issues. The Case Manager will serve as theprimary point of contact for all assigned client services and provide guidance and support to clients working towardself-sufficiency.How you will contribute:Perform assessment of each family's housing history and barriers, physical, mental health and social needs, educational and employment needs, financial management skills, strengths and needs, and other necessary areasMeet with assigned families regularly and provide appropriate services and referrals geared towards housing stabilityAssure all relevant information pertaining to clients is disseminated to all appropriate partners in a timely mannerSupport clients in the creation and maintenance of financial budgets and support clients in implementing money management techniquesParticipate in meetings with representatives from other agencies including Faith-Based and Community Based Organizations to deliver wrap-around services to clientsEnsure that clients achieve the objectives of maintaining long-term housing, by complying with the terms of their lease agreement and have an Individualized Housing Sustainability Plan that helps them achieve their desired level of self-sufficiencyMediate household, roommate, and/or landlord-tenant disputes that threaten housing stabilityServe as point-of-contact for landlords and maintain positive landlord relationshipDevelop and maintain a Resource Directory that includes state and local service providersMaintain thorough case documentation in HMIS and client files and comply with agency and standard best practicesProficiently utilize Google Suites applications (email, calendar, drive, docs, etc) as a part of day to day work requirementsFunction as a contributing member of a multidisciplinary team with Shelter House Inc. employees deployed staff and involved agenciesExemplify the Shelter House core values; Collaboration, Accountability, Respect and EmpowermentCarry out other duties as assigned by Assistant Director and/or Program DirectorPrepare monthly reports to Supportive Services ManageRequirementsRequired:Bachelor's Degree in Social Work, Psychology, Human Development, or related degree or commensurate experienceThree years of experience in service delivery with low and moderate-income familiesAbility to prioritize competing priorities and make sound judgmentStrong written communication skillsAbility to advocate, assist in problem-solving, and achieve results for familiesExcellent time management and organizational skillsValid Driver's LicenseWillingness to work evenings and weekends as neededPreferred:An MSW or master's degree in human services/ related fieldExperience with providing services to clients experiencing homelessness, Mental Health or substance abuse issuesAbility to speak a language other than English at a proficient level (i.e. Spanish, Farsi)Comfortably with and quickly adapts to changeStrong public speaking and communication skillsPhysical Requirements: Annual TB Test is required Ability to sit or stand for long periods Ability to lift items weighing 10-20 pounds BenefitsBenefits:Medical, Dental & Vision Insurance 401K contributions with a 4% employer match 13 Paid Holidays, 2 Floating Holidays and the opportunity to take your Birthday Off Two Semi-Annual Team Building Events Equal Employment Opportunity:Equal Employment Opportunity Policy: Shelter House is committed to providing equal employment opportunity to all employees and applicants.
