Position Summary The Case Manager provides non-clinical, guest-centered case management services to individuals experiencing homelessness. The primary focus of this role is addressing Social Determinants of Health (SDOH) by assisting guests in obtaining identification, benefits, housing resources, and connections to community-based services. The Case Manager works collaboratively with guests to assess needs, develop individualized service plans, coordinate services, and advocate with external systems. This role serves as a liaison between guests and outside agencies, supporting progress toward housing stability, benefits access, employment and self-sufficiency. Scope of Practice, Boundaries and Clinical Supervision · Provides case management and care navigation services only · Does not provide therapy, diagnosis, clinical assessments, or treatment · Focuses on assessment, planning, coordination, referral, advocacy, and follow-up · Works in collaboration with clinical, peer specialists, QBHA, and operational staff Primary Responsibilities Assessment and Service Planning · Conduct intake and ongoing assessments to identify guest needs, strengths, and barriers · Assess Social Determinants of Health, including housing, income, benefits, healthcare access, legal status, and transportation · Develop and maintain individualized, guest-centered service plans · Review and update service plans based on guest progress and changing needs Case Management and Follow-Up · Provide ongoing case management through scheduled guest meetings and check-ins · Track progress toward goals and document outcomes · Support guests in navigating systems and completing required steps independently when possible · Maintain engagement and follow-up with guests to reduce service gaps Benefits, Identification and Documentation Assistance · Assist guests in obtaining and maintaining essential documentation, including: · Birth certificates · Social Security cards · State identification · Medicaid and healthcare coverage · SNAP / food benefits · Support completion, submission, renewal, and follow-up of benefit applications · Coordinate with benefit offices to resolve delays or denials Resource Coordination and Advocacy · Serve as a liaison between guests and external service providers, including: · Housing and shelter programs · Government agencies · Healthcare and behavioral health providers · Workforce and employment services · Legal aid organizations · Make referrals and follow up to confirm service access · Advocate on behalf of guests with landlords, agencies, and service providers Housing Support · Assist guests with housing applications, documentation, and referrals · Support compliance with housing program requirements · Coordinate with housing partners to support placement and retention Documentation and Compliance · Maintain accurate, timely, and complete case notes · Document assessments, service plans, referrals, follow-ups, and outcomes · Ensure documentation meets funding, grant, and regulatory requirements · Use electronic record systems in compliance with privacy and confidentiality standards Collaboration and Professional Conduct · Participation in team meetings and case conferences as required · Communicate effectively with internal staff and external partners · Maintain professional boundaries and ethical conduct · Follow organizational policies and procedures Qualifications Required: · Bachelor’s degree in human services or related field · Minimum one (1) year experience providing case management or social services to individuals experiencing homelessness or housing instability · Knowledge of Social Determinants of Health and community resources · Strong organizational, documentation, and advocacy skills · Basic computer proficiency and experience with electronic case management systems · Ability to pass required background checks · Ability to pass urine drug screen Preferred Qualifications · Experience in shelter, day center, or housing programs · Familiarity with Medicaid, SNAP, SSI/SSDI, and housing systems · Experience coordinating services across multiple agencies · Bilingual or culturally specific competencies Work Environment and Physical Requirements · Primarily shelter based with some community coordination · Ability to walk, stand, and move throughout the facility · Ability to work in a fast paced, high needs environment · Occasional evening or weekend hours as required Job Type: Full-time Pay: $60,000.00 - $65,000.00 per year Benefits: 401(k) 403(b) 403(b) matching Dental insurance Employee assistance program Health insurance Life insurance Paid time off Retirement plan Vision insurance Work Location: In person
