Family Nurse Practitioner
Job Summary
Provides direct primary care to adult and geriatric patient populations who reside in Residential Care Facilities, Assisted Living, Memory Care Facilities, and more. The NP will provide comprehensive healthcare services to a diverse patient population, focusing on prevention, diagnosis, and treatment of acute and chronic conditions. The Nurse Practitioner will deliver services under the direction of the interdisciplinary team and Chief Medical Officer or Medical Group Physician.

Supports the philosophy, objectives and mission of BeLoved Hospice.
Stays current with all mandatory agency requirements.
Attire to be consistent with the BeLoved Hospice dress code policy.
Specific to Position:
Conduct thorough physical examinations and health assessments
Develop and follow individualized Plan of Care for each patient
Diagnose and treat acute and chronic health conditions, including prescribing medications and managing ongoing therapies.
Acts as an expert consultant or attending physician for primary care patients as assigned
Assists the Chief Medical Officer and/or Family Medicine Physician in the oversight of patient care by providing direction for orders and interventions to optimize patient outcomes.
Performs face-to-face visits for patients within a specified time frame.
Capable of giving adequate report on patient conditions to the Chief Medical Officer, Medical Director, RNs and other members of the interdisciplinary team, as needed.
Reports unusual conditions, observations and pertinent information to the correct interdisciplinary team members.
Provide health education and counseling to patients, families, and facility staff on disease prevention, lifestyle modifications, and self-management.
May participate in the competence evaluations of nursing staff and other nurse practitioners.
Participates in community education, as requested, by the Administrator, Director of Nursing or Chief Medical Officer
Shares in the physician on-call rotation, with back-up available at all times from the medical group physicians.
Maintain accurate and up-to-date patient records in accordance with legal and ethical standards
Order, interpret, and follow up on diagnostic tests and laboratory results
Serves as a source of clinical expertise to facility staff, agency, providers, and payer sources.
Acts as a resource for prescriptions when patient’s attending physician is not available.
May perform or assist in medical procedures at the patient’s residence, including intravenous or subcutaneous line placement, and phlebotomy.
Maintains in-depth knowledge of pharmaceuticals for management of patient symptoms
Participates in interdisciplinary team meetings
Participates in Performance Improvement activities as requested.
Consults appropriately with Chief Medical Officer or Medical Group Physician if uncertain about management of a disease, treatment options, or if required interventions beyond the knowledge and expertise of the nurse practitioner.
Provides emotional and spiritual support and makes appropriate referrals to other team members
Facilitates communication among patient, family and hospice team
Other duties as assigned.
Physical/Safety Requirements:
Participates in emergency procedures, including disaster drills.
Operation of equipment in a safe manner and shares responsibility for communicating safety violations or unsafe practices to management.
Equipment Used: General office equipment (telephone, copy machine, computer, fax machine) and motor vehicles.
Category 1 for potential exposure to blood/bodily fluids.

Qualifications and Experience
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required to perform this job.
Must be hold active certification as a Geriatric Nurse Practitioner or Family Nurse Practitioner through the American Academy of Nurse Practitioner (AANP) or American Nurses Credentialing Center (ANCC).
Maintains active NP and RN licensure with the Oregon Board of Nursing.
Maintains Basic Life Support (BLS) Certification.
Maintains active NPI and DEA registration and provides such information to the Medical Group
Prefer prior experience in family medicine or geriatrics.
Must be able to speak, read and write the English language.
Must have reliable transportation and maintain auto insurance.
Successful completion of background check pursuant to ORS 181A.195.
Must be able to handle frequent interruptions, problem solve, and have exceptional follow through skills.
Must be able to cope with emotionally charged situations encompassing patients, families, caregivers, and team members.

Pay: $120,000.00 - $150,000.00 per year
